The workspace hit a plan-level quota (sends, contacts, webhooks).

What happened

Plan quotas are enumerated at `GET /v1/workspace`. Soft limits (80% of monthly sends) emit `usage.threshold_reached` to webhooks; hard limits return this code.

How to recover

Upgrade the plan, archive unused resources, or wait for the period to roll over.
